Emergency Calls

If you have an emergency during office hours, please call our office and state the nature of your problem. If we are closed, please go directly to Troy Regional Medical Center’s Emergency Room. Although Dr. DiChiara is on call most of the time, he will occasionally ask another doctor to cover his practice. The hospital operator will know how to contact the physician on call. To reach Dr. DiChiara or the on-call doctor after hours or during weekends and holidays, please call 334-670-5000.

Office Appointments

We will always try to honor your appointment time. Please understand, however, that emergency patients may cause unforeseen delays. If you are unable to wait, notify the receptionist, and she will be happy to reschedule you for another time.

Please notify the office as soon as possible if you are unable to keep an appointment. A 24-hour notice is preferred. If you are a new patient to our office, please arrive several minutes ahead of your scheduled appointment to complete your forms. Please bring a list of current medications, including names, dosages and how frequently each is taken, along with any allergies you may have. Also, have your insurance cards and/or forms with you.

Medication Refills

All prescriptions and authorizations for renewals should be requested before 10 a.m. if possible. Prescriptions will be written or called in around 4 p.m. Certain medical problems may require evaluation in the office before any new prescriptions can be written.

Confidentiality

All of your medical records are confidential. No information will be released or discussed with any other person without your prior written authorization.

Fees, Bills and Collection

Due to the high cost of billing and most insurance requirements, we ask for payment at the time of service. If you have a special financial problem, please let us know in advance so that arrangements can be made for payment.

Physician fees are directly related to the value of time devoted to the diagnosis of the patient’s illness, to its management and to the complexity of judgment required in the case.

Insurance Claims

We are providers of Medicare, Blue Cross/Blue Shield PMD PPO, Medicaid and most other major insurance carriers and group plans. Please ask our receptionist when you make your appointment if Dr. DiChiara participates in your particular insurance plan. We ask that you be prepared to assume responsibility for deductibles and co-pays at the time of service.

Remember that insurance is considered a method of reimbursing the patient for fees paid to the doctor and is not a substitute for payment. Some companies pay fixed allowances for certain procedures, and others pay a percentage of the charge. It is your responsibility to pay any deductible amount, coinsurance or any other balance not paid for by your insurance company.

If you don’t have insurance or need to set up a payment plan to meet your obligation, please call prior to your scheduled visit, and we can give you an estimate of charges.
